When working with your child at our initial assessment several pieces of information will be collected in order to get an overall idea of what is happening at the time.

When someone fills out the initial consultation forms, I ask for details such as the child’s name, parents or caregivers name, address and contact number, as well as details of the family GP, school and general health information.
-
I also keep brief notes on each session. These notes are a short factual summary only, using you childs initials, these notes will summarise what was said and done during the session, and are completely confidential. I keep notes so that I can look back and remember what has been covered in our session.

I will never sell your data or use it for any purpose we have not agreed. In the unusual event that I have been explicitly directed by a court order or a coroner request, I will only share the minimum necessary information. Where possible, I would get your explicit consent before sharing any data.
